UKHSA, through ImmForm, supplies products classed as `Medicinal Products’ which are regulated by the MHRA. As part of the Human Medicines Regulations 2012 we are required to undertake customer validation checks therefore we reserve the right to restrict supply of Medicinal Products to validated customers only. If you order products from us, the professional registration(s) of the individual accountable for the medicines upon delivery will be subject to an initial validation at the time of account application. Wholesale Dealers will be required to provide their Wholesale Dealer Licence number, WDA and GDP. These data will subsequently be re-validated in the form of a request for information. This will enable us to qualify each account as a valid legal entity. Staff who order vaccines should ensure they meet all necessary legal requirements for the subsequent possession of vaccines, which are prescription-only medicines (POMs). This will be covered when they are acting on behalf of a registered medical practitioner with a licence to practice or because the use of the vaccine is authorised through a patient group direction.
We collect information about you in accordance with our Privacy Policy and our Cookie Policy. By using IMMFORM.PHE.GOV.UK, you agree to us collecting this information and confirm that any data you provide is accurate. Organisation ordering data, excluding personal data, may be shared with NHS England or NHS Business Services Authority (NHSBSA) at the request of and overseen by NHS England for use as part of verification processes, investigation or measurement exercises.
